目的 比较膝关节单髁表面置换术(unicompartmental knee arthroplasty, UKA)治疗膝前内侧骨关节炎伴无症状外侧半月板损伤与无外侧半月板损伤的前内侧骨关节炎的临床疗效,评估术前磁共振提示外侧膝关节半月板损伤对前内侧骨关节炎患者UKA术后功能的影响。方法 回顾性研究2022年1月至2022年12月在温州医科大学附属第二医院UKA治疗无症状外侧半月板损伤的膝前内侧骨关节炎15膝(LP组)和无外侧半月板损伤的膝前内侧骨关节炎15膝(LN组),观察随访期间并发症发生的情况,比较两组术前体征、影像学资料和手术情况。采用美国特种外科医院膝关节评分(Hospital for Special Surgery Knee Score, HSS score)对术前、术后膝关节功能进行评估。记录末次随访时膝关节活动度(range of motion, ROM)和膝关节外侧疼痛程度。结果 所有患者均获得了完整的随访,随访时间为12 ~ 16个月,平均(13.9±1.8)个月,随访期间均无明显并发症出现。两组患者手术时间平均为(61.2±8.7)min,使用Oxford Ⅲ假体。两组在膝关节HSS评分、ROM和膝关节外侧疼痛方面比较,差异无统计学意义(P>0.05)。结论 对于膝前内侧骨关节炎伴无症状的外侧半月板损伤的患者,UKA不会加重外侧半月板损伤的相关症状,短期临床疗效良好。 |

Objective To compare the clinical efficacy of unicompartmental knee arthroplasty (UKA) in the treatment of anteromedial knee osteoarthritis with asymptomatic lateral meniscus injury and anterior medial anteromedial knee osteoarthritis without lateral meniscus injury, and evaluate the effect of preoperative magnetic resonance imaging (MRI) findings of lateral knee meniscus injury on the function of patients with anterior medial anteromedial knee osteoarthritis after UKA.Methods A retrospective study was conducted from January 2022 to December 2022 on 30 patients who underwent (UKA) at the Second Affiliated Hospital of Wenzhou Medical University. Among them, 15 patients (15 knees) suffered from anterior medial anteromedial knee osteoarthritis with asymptomatic lateral meniscus injury (LP group) and 15 patients (15 knees) suffered from anterior medial anteromedial knee osteoarthritis without asymptomatic lateral meniscus injury (LN group). The incidence of complications during follow-up was observed, and the preoperative clinical signs, imaging data and operation conditions were compared between the two groups. We used the Hospital for Special Surgery Knee Score (HSS score) to evaluate knee function before and after surgery. The knee range of motion (ROM) and lateral knee pain were recorded at the last follow-up.Results All patients were followed up for 12-16 months, with an average of (13.9±1.8) months, and no significant complications occurred during the follow-up. The mean operation time was (61.2±8.7) min, and Oxford Ⅲ prostheses were used in all patients. There were no significant differences in knee HSS score, ROM and lateral knee pain between the two groups (P>0.05).Conclusion For patients with anteromedial knee osteoarthritis with asymptomatic lateral meniscus injury before surgery, the implementation of UKA will not aggravate the symptoms related to lateral meniscus injury, and the short-term of UKA clinical effect is good. |
