关节镜下松解术治疗冻结肩疗效观察

目的观察关节镜下粘连松解术治疗冻结肩的临床疗效。方法2014年1月至2015 年12月，对23例冻结肩患者，原发性18例、继发性5例，进行关节镜下粘连松解。术前、术后根据VAS 疼痛评分及Constant肩关节功能评分评价治疗效果。结果 23例患者全部随访，平均随访16个月(12～30个月)。在术后1月，3月，12月时疼痛评分2.5±0.95, 1.3±0.54, 1.0±0.42分，较术前6.5±2.31明显降低，肩关节功能评分78.6±4.61，80.6±3.99，85.5±4.45分，均高于术前30.6±3.63，差异具有统计学意义（P<0.05）。结论采用关节镜下粘连松解治疗原发性及继发性冻结肩，创伤小，恢复快，疗效可靠。

Objective To observe the clinical effect of arthroscopic capsular release treatment of frozen shoulder. Methods From January 2014 to December 2015, 23 cases of frozen shoulder patients, including primary 18 cases, secondary 5 cases were treated with arthroscopic capsular release. The VAS pain score and Constant shoulder function score were used to assess the treatment effect. Results All the patients were followed up for an average of 16 months (12 to 30 months). At 1 month, 3months, 1 year after surgery, the pain VAS score were 2.5±0.95, 1.3±0.54, 1.0±0.42 which were significantly lower than preoperative which was 6.5±2.31，the difference was statistically significant (P<0.05). The score of the shoulder joint function were 78.6±4.61, 80.6±3.99, 85.5±4.45 at 1 month, 3 months, 1 year after surgery which were higher than preoperative which was 30.6±3.63, the difference was statistically significant (P<0.05). Conclusion The treatment of frozen shoulder by arthroscopic capsular release is an ideal method, which has the advantages of rapid recovery, less trauma, fewer complications.
