Objective To explore the effects of desmopressin acetate on perioperative indexes and coagulation function in intertrochanteric fractures after intramedullary nail (Intertan) internal fixation.Methods A total of 90 elderly with intertrochanteric fractures admitted between January 2018 and January 2021 were enrolled in Department of Orthopedics, Shandong Provincial Hospital Affiliated to Shandong First Medical University. They were randomly divided into observation group (n=45) and control group (n=45). Observation group: 250 mL normal saline injection and 150 μg desmopressin acetate (1 mL: 15 μg; Shenzhen Guangdong; National medicine approval H20064094), 200 mL acetate desmopressin diluent was injected into the medullary cavity from the insertion point, and the incision was soaked with the remaining acetate desmopressin diluent for about 5 min before suture. Control group: The same method was used to give the same amount of normal saline injection as control. The perioperative blood loss and coagulation function of the two groups were observed. The blood loss, blood transfusion volume, hemoglobin, hematocrit, coagulation function and occurrence of complications were compared between the two groups.Results The intraoperative blood loss, hidden blood loss, total blood loss, drainage volume at 1, 2 and 3 d after surgery, total drainage volume, blood transfusion volume and blood transfusion rate in observation group were lower than those in control group (P<0.05), hemoglobin and hematocrit at 1 d and 3 d after surgery were significantly better than those in control group (P<0.05). Before surgery, at 1 d and 3 d after surgery, there was no significant difference in prothrombin time, international normalized ratio, activated partial thromboplastin time or D-dimer between the two groups (P>0.05).Conclusion The application of desmopressin acetate in patients with intertrochanteric fractures after Intertan internal fixation can effectively reduce perioperative blood loss and blood transfusion volume, which will not affect coagulation function or increase postoperative complications, with high safety. |
